Patient Portal

Our office has implemented the use of the patient portal.  Please contact us if you are an active patient in our office and would like to register on the patient portal.  We will provide you with the necessary activation code.

Some of the benefits of having an active portal account include: being able to request appointments or schedule your own follow-up appointment, view your upcoming or previous appointments, request medication refills, view your chart and print a summary of it, and make changes to your account and medical information.  You may also message our office if you have any non-urgent questions.

Electronic Medical Records System

 

In an effort to continue to improve your quality of care, Allentown Asthma & Allergy has transitioned to electronic medical records (EMR).

We ask for your assistance in the following ways:
For your first visit, whether it is for allergy injections or an office visit to see the doctor, please bring your current insurance card and current photo ID with you. These items must be scanned into the new system. It is important that you present your current insurance card to ensure your claims are filed properly.

 

You will also be asked for additional personal information that we currently do not capture. This information is necessary for us to meet government-established guidelines for our electronic medical record system. The required information will include race, ethnicity, and primary language.

We will also be asking for your pharmacy names and telephone numbers.

Office Details

 

Appointment times are available Monday through Friday with two evenings available.  We also open at 7 a.m. Wednesday mornings for those early risers!

Nurse Calls - we encourage our patients to contact us in the morning if you need to speak with a nurse.  This allows us time to speak with the provider and return your call.

Prescriptions - We now offer e-prescribing!  Your prescription will be sent electronically - much quicker and more efficient!

 

Note: Please allow 24 hours for prescriptions to be called in to your pharmacy - a pharmacy phone # is required.  Written prescriptions may be picked up or you may prefer to provide us with a self addressed stamped envelope.

 

> Insurance Coverage

We participate with most insurance plans.  It is the patient's responsibility to know their coverage, deductibles and co-pays.  Patient's whose plans require referrals need to obtain one prior to their appointment.  Co-pays are due at the time of service.

 

School Forms - school forms are completed at no charge at a patient's appointment, otherwise there is a $10.00 charge for completion. Parents may pick up the forms when done or provide us with a self addressed stamped envelope.
